The throat — technically the pharynx and larynx, together with the tonsils, adenoids, and the soft tissues of the upper airway — is one of the three principal areas of ENT practice alongside the ear and nose. Throat conditions range from recurrent tonsillitis in childhood to obstructive sleep apnoea in adults, and the surgical management of these conditions is a core part of Dr Roth’s general ENT practice. Tonsils & Adenoids

Tonsillectomy and adenotonsillectomy are among the most commonly performed ENT procedures in both children and adults. Indications include recurrent tonsillitis (meeting the Paradise frequency criteria), peritonsillar abscess, obstructive sleep apnoea from tonsillar and adenoid hypertrophy, and chronic tonsillitis with tonsil stones and persistent halitosis. Adenoidectomy alone addresses adenoid-related glue ear and nasal obstruction. Snoring & Sleep Apnoea — Adults

Snoring exists on a spectrum from simple positional snoring to severe obstructive sleep apnoea (OSA). OSA is associated with cardiovascular disease, metabolic syndrome, impaired cognition, and significantly increased road accident risk — with Australian driving legal obligations for sufferers. Assessment involves a full upper airway examination and, in most cases, a sleep study. Treatment options range from positional therapy and weight management through to CPAP, mandibular advancement splints, nasal surgery, UPPP, and hypoglossal nerve stimulation (Inspire). The nasal component of OSA is frequently addressed simultaneously with throat surgery.

Snoring & Sleep Apnoea — Children

Snoring in children is a distinct and more serious condition than in adults. Paediatric OSA causes measurable neurodevelopmental deficits in attention, memory, and learning, and is a significant cause of misdiagnosed ADHD. Growth hormone is secreted during deep sleep — OSA disrupts this in some children. The most common cause is adenotonsillar hypertrophy, and adenotonsillectomy is the standard first-line surgical treatment. Why See a Specialist Otolaryngologist for Throat Problems?

Many throat conditions are initially managed by a GP — and appropriately so. Referral to a specialist is warranted when conditions are recurrent or persistent, when the frequency of tonsillitis meets the criteria for surgical consideration, when snoring is associated with witnessed apnoeas or daytime sleepiness suggesting OSA, when there is concern about a unilateral tonsillar asymmetry, or when a child’s snoring is affecting sleep quality, behaviour, or school performance.

As a Specialist Otolaryngologist, Dr Roth has specific training in the diagnosis and surgical management of the full range of throat conditions. He assesses throat problems in the context of the whole upper airway — including the nose, which contributes significantly to both snoring and OSA — and is experienced in both adult and paediatric throat surgery. Co-located audiology services at the Dee Why consulting rooms allow hearing assessment to be performed at the same visit for patients whose throat conditions are associated with ear symptoms.

Whole Upper Airway Assessment

Snoring and sleep apnoea frequently have both a nasal and a throat component. Dr Roth assesses and manages both simultaneously — nasal surgery combined with throat surgery in a single anaesthetic where indicated — rather than addressing each in isolation.
